In regard to alarm times-
I think it might be a lot more efficient to create a tap menu that would display increments of 0, 5, 10, 20, 30 etc... for minutes. When the user selects hours, the increments could change to 0, 1, 2, 3, etc...
0 would stand for current appointment time.
Tapping the up and down arrows is tedious and I doubt that anyone ever needs all of those options. Most people need to be notified in increments of 5 or 10 min not 11, 12, 13, 14 etc...
The alarm times can still be completely customizable. If the user wanted another increment (say 19 minutes before or something like that) they could just simply type it in the box.
